Medicare Chronic Disease Management (CDM) Plans
Some clients may be eligible for Medicare rebates through a Chronic Disease Management (CDM) Plan, previously known as an EPC.
What is a CDM Plan?
A CDM Plan is prepared by your GP and provides up to 5 Medicare-subsidised allied health appointments per calendar year for people with chronic or complex conditions.
How the Rebate Works
Medicare provides a partial rebate per session.
A gap payment still applies.
A valid GP referral is required before your appointment.
What Conditions Are Eligible?
Your GP determines eligibility, but common examples include:
Chronic (longer than 3 months) musculoskeletal pain
Long-term injuries
Post-operative recovery requiring ongoing care
Pelvic health conditions
Persistent neurological or mobility conditions

Private Health Insurance & HICAPS
You can choose to use HICAPS and claim on the spot for your physiotherapy sessions through private health insurance if you hold Extras cover.
How It Works
Physiotherapy and Clinical Exercise (Pilates and Strength and Conditioning classes) are covered under Extras insurance.
The rebate amount varies depending on your fund and level of cover.
You pay only the gap (the difference between the fee and the rebate amount).

We have HICAPS facilities in clinic, allowing you to:
Claim your rebate immediately after your appointment
Only pay the remaining balance
Avoid needing to submit receipts or wait for reimbursement
Services
